An offside occurs when an attacking player gets closer to the goal line of the opponent than the ball or the last defender.
FIFA rules state that a player can be penalized if he is offside when the ball is touched or played to by one of his teammates.
This means that all of a player's head, feet and body must be in front the last defender when they pass the ball to them. They will be expelled if they don't.
Referees can award an indirect free kick to opposing teams if they determine that a player has been offside. This is conceptually identical to a regular free kick. However, the ball must be handed to another player before a goal is possible.
A player can't be expelled if they are given the ball via a corner kick, throw-in, or kick-in.
These are great calls. We live in an imperfect world. Referees may miss a goal scorer or deny a goal that was deserved. This is where the fans scream and cry in the stands, making football a soap opera.

If an opponent deliberately blocks the ball, an offside player who gets the ball from him will not be considered to have gained an edge.
"Save" refers to when a player tries to stop a ball from entering the goal or is very near the goal with any part or body other than the arm or hand (unless the goalkeeper's in the box).
On a set piece, or a free kick, it's offside:
The above image shows a "setpiece" getting ready to play. The red circle player is getting ready for the ball to be passed to the middle of the box (where is the blue arrow pointing). He wants to see one of his players hit the ball with the head to place the ball in the net. The ball is kicked and four players of the blue team are past the last defender (marked at the yellow line). The play will be ruled offside if any of these players touch the ball.
FIFA, the official soccer governing body in the world, states that a player can actively interfere with play by touching the ball after it is passed to them. A player can still interfere with play by touching the ball. If the referee believes that the offside position of the player is in conflict with the opponent, he will be penalized. By preventing another player from playing the ball. Oder obstructing the goalkeeper's view.
